CAP REVIVAL CARE LLC is duly licensed by the Agency for Health Care Administration (A.H.C.A.), Florida License #: 240593to provide Homemaker and Companion Services throughout Hillsborough and Manatee Counties.

Furthermore, CAP REVIVAL CARE LLC is an approved provider for the Florida Agency for Persons with Disabilities (APD) and participates in both the Home and Community-Based Services (HCBS) Medicaid Waiver Program and the Developmental Disabilities (DD) Medicaid Waiver Program in accordance with applicable Florida statutes, APD rules, and regulatory requirements.

In addition, CAP REVIVAL CARE LLC is a credentialed Medicaid provider serving members enrolled through Florida Community Care (FCC) and Aetna Long-Term Care (LTC) Medicaid plans and programs, further expanding access to high-quality community-based support services.

Our team of caregivers, consisting of Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), and certified professionals, undergoes extensive training to accommodate the unique needs of individuals facing challenges such as Intellectual Disabilities, Hearing Impairment, Autism, ADHD, Physical Disabilities, Visual Impairments, Dementia, Alzheimer’s and or other cognitive impairments as well as companionship, personal support, and or respite care to individuals in need. We focus on understanding each client’s specific needs to provide personalized care that enhances their quality of life.

Position

Direct Care Staff support individuals with developmental disabilities by promoting independence, safety, and quality of life. Responsibilities include assisting with activities of daily living (ADLs), providing medication reminders (non-administrative), implementing Individual Service Plan (ISP) goals, and encouraging personal growth through person-centered care. They foster dignity, self-sufficiency, and community involvement while meeting each individual's unique needs.
